After scheduling your appointment,
please fill out the following forms prior to your
consultation
Having the appropriate paperwork completed
in advance assists us in providing you with efficient
health care. Please complete as much of these
forms as possible as they are required for your
consultation. Any questions you may have about
the forms will be addressed by our office at the
time you present for your first visit.
When bringing in your forms, please
make sure you include all of the following:
- Assignment
and Release/ Patient Information Form
(Signature Required)
- Medication
Log
- Physician
List (Signature Required if records
will be mailed to another physician)
- Patient
Questionnaire (8 pages) or
Pediatric Patient Questionnaire (8
pages)

If you have had a sleep study, it
would be helpful to have that on file before your
consultation. You can contact one of our staff
members at (512) 329-9296 and they can help you
obtain your prior sleep study.
If you are currently undergoing treatment
for sleep apnea and are currently using a CPAP machine,
please bring that machine with you to your first
appointment.
Also, don’t
forget to bring your insurance card(s) to your first
visit.
